Homeless Outreach Case Manager (Hybrid), $24 hr + $1,500 Sign-on Bonus!

Kennebec Behavioral Health is currently seeking a dedicated case manager to join our Homeless Outreach team (Hybrid Position – In-person work in the community required with the option to complete administrative tasks in home office or one of our KBH locations!) This position would be traveling throughout Androscoggin and Franklin counties.

$1,500 Sign-on Bonus opportunity available!

The principal role of the Homeless Outreach Case Manager is to provide outreach, engagement and referral resources to the homeless population who also experience serious mental illness or co-occurring serious mental illness and substance abuse disorders. The Homeless Outreach Case Manager utilizes strategies aimed at engaging persons into the needed array of services, including identification of individuals in need, screening, development of rapport, offering support while assisting with immediate and basic needs and referral to appropriate resources.

Requirements – B.A. or B.S. in Social Services, Mental Health or a related field is required. MHRT/C or MHRT/C provisional required. Experience with billable case management preferred. Valid Driver’s License required. This position is well-suited for someone who is comfortable working independently and utilizing technology.
